Personal

• Name, social security number and birth date of all borrowers
• Address for the past 24 months

Employment

• Employer(s) for past 24 months to include name, address and phone number

Income

• W-2’s for the most recent 2 years
• Pay stubs that cover the most recent 30 days
• Self-Employed: defined as 25% or more ownership
  • Most recent personal tax return to include all pages and schedules
  • Corporations / Partnerships
    • Most recent corporate or partnership return to include all pages and schedules
    • Year to date profit and loss (Unaudited)

Assets

• Bank statements or deposit accounts that cover the most recent month or most recently quarterly statement (Be sure to include all pages)
